What are features of PCOS?
Subfertility/infertility
Menstrual disturbances- oligomenorrhoea/amenorrhoea
Hirsutism e.g. acne
Obesity
Acanthosis Nigricans
What investigations are required?
Pelvic USS- multiple cysts
Prolactin- normal/mildly elevated
LH:FSH- can be raised
Testosterone- normal/raised
Sex hormone Binding globulin- low
Check for impaired glucose tolerance
How is it diagnosed?
Rotterdam Criteria
2 of 3 present
1) Infrequent or no ovulation
2) Clinical or biochemical signs of hyperandrogenism
3) Polycystic ovaries in one or both ovaries >12 follicles
What is the management?
Weight loss
COCP can be good for acne/regulating cycle
Fertility
- specialist review
- clomifene can be good
- Metformin second line
