What is it?
A
Rotation of the ovary and part of the fallopian tube
Obstructs blood flow leading to oedema and arterial compromise
Leads to ischaemia which if not reversed causes ovarian necrosis

What are the symptoms?
A
Sudden onset pelvic pain
- unilateral affecting RIF/LIF
- pain worsens over time
- intermittent if ovary untwisting
N&V
May find palpable mass
Abdo tenderness with guarding

What investigations are required?
A
Urgent transvaginal US- whirlpool sign

Q
What is the management?
A
Surgical intervention
- emergency surgery- laparoscopy
- may require oophorectomy if ovary non-viable
