Diffuse axonal injury MOA?
Occurs as a result of mechanical shearing following deceleration, causing disruption and tearing of axons
Classification of head injury?
- Focal = contusion/haematoma

Classification of SAH?
- Traumatic
2. Spontaneous
Causes of spontaneous SAH?
- Berry aneurysms (85%)
- AVM
- Pituitary apoplexy
- Arterial dissection
- Mycotic (infective) aneurysms
Ix of SAH?
- CT = Acute blood (hyperdense/bright on CT) is typically distributed in the basal cisterns, sulci and in severe cases the ventricular system.
- LP = used to confirm SAH if CT -ive, at least 12 hours after onset of sx to allow development of xanthochromia
Ix after a spontaneous SAH is confirmed?
CT cranial angiogram
Complications of aneurysmal SAH?
- Re-bleeding
- Vasospasm
- Hyponatraemia due to SIADH
- Seizures
- Hydrocephalus
- Death
Vasospasm post SAH Mx?
21 day course of nimodipine (CCB targeting brain vasculature)
Most sensitive scan for DAI?
MRI
Blown pupil after head trauma
Transtentorial herniation causing third cranial nerve compression
Binocular vision post-facial trauma?
Depressed fracture of the zygoma
Hyphaema defn?
Collection of blood inside the anterior chamber of the eye, between the cornea and iris
Mx of extradural haematoma?
Parietotemporal craniotomy
What drug should be prescribed for SAH?
Nimodipine to prevent vasospasm
Criteria for brain stem death testing?
- Deep coma of known aetiology
- Reversible causes excluded
- No sedation
- Normal electrolytes
