How does pain perception correlate to education level?
Higher educated individuals have less pain, pain catastrophizing, and distress
Correlation between nicotine and pain perception?
Positive correlation
What is reflexive pain?
Reflexive pain occurs at the level of the spinal cord
What is conscious pain?
Semi conscious pain that occurs at the level of the limbic system and cortex
What is transference?
Patients assume that you can’t or won’t help them and that yo will likely not listen/ be rude
What is countertransference?
Physician will likely be stressed, irritated, short and rude as these patients dont follow medical rules
Name some nonaddictive interventions for pain?
NSAIDS, Exercise/ PT, acetaminophen, TCAs, antidepressants, AEDs, therapy, massage, regional nerve block
Name some interventional approaches for pain?
Nerve blocks, Facet injection, Epidural, radio frequency ablation, dorsal column stimulator, pain pump
Name some non Rx and non interventional approaches for pain?
TENS, PT, Aqua therapy, Massage therapy, CBT
What are the important factors when considering a pain procedure (intervention) for a patient?
Must understand underlying condition
Must ensure a primary pathology is treated
Must understand the patient (depression, somatic disorder druct history, ect)
What are neurectomy and ganglionectory-rhizotomy mainly used for?
Cancer/ Malignant pain becuase it helps for a while but then you develop neuropathic pain after some time
What is a cordotomy used for?
Unilateral pain, but the effect wears off after a while
What is midline myelotomy used for?
Visceral pain
What are midline thalotomies and cigulate cortex lesions used for?
Emotional pain?
What is the basis and use of neuroblative procedures?
Used for back pain of facet origin
