Considerations in paediatric prescribing
A
- Age and DoB
- Current body weight (check appropraiteness of written BW)
- Body surface area
- Drug allergy
- G6PD status
- Current drug profile review: avoid overdose, D-D interaction, Therapeutic duplications…etc
- L/RFT: eGFR with bedside Schwartz Formula (Not using Cockcroft and Gault for adults)
- Decide dose regimen: dose, frequency, duration, route…etc; Dose per unit mass/ BSA (mg/kg or mg/m^2)
- Therapeutic drug monitoring: clinical resolution, side effect (esp. Vancomycin, Digoxin, Lithium… with narrow therapeutic index)
- Side effects: Nephrotoxicity, Hepatotoxicity, D-D interaction, Drug- Disease interaction
- Chemical compatibility
