Considerations in paediatric prescribing

A
  1. Age and DoB
  2. Current body weight (check appropraiteness of written BW)
  3. Body surface area
  4. Drug allergy
  5. G6PD status
  6. Current drug profile review: avoid overdose, D-D interaction, Therapeutic duplications…etc
  7. L/RFT: eGFR with bedside Schwartz Formula (Not using Cockcroft and Gault for adults)
  8. Decide dose regimen: dose, frequency, duration, route…etc; Dose per unit mass/ BSA (mg/kg or mg/m^2)
  9. Therapeutic drug monitoring: clinical resolution, side effect (esp. Vancomycin, Digoxin, Lithium… with narrow therapeutic index)
  10. Side effects: Nephrotoxicity, Hepatotoxicity, D-D interaction, Drug- Disease interaction
  11. Chemical compatibility
