Linezolid (zyvox)
Linezolid MOA
Binds to ribosomal 50s subunit, prevents formation of 70s initiation complex which inhibits protein synthesis
Linezolid Susceptible Organisms
Aerobic, Gram-Positive
VRE, MRSA, Streptococcus
group A/B StrepS. pneumoniae, E. faecalis, E. faecium, S. Aureus MRSA/SA
Linezolid Absorption
Rapid/complete after oral admin
Linezolid Distribution
31% Protein Bound - Conc. Dependent
High concentration found in skin and well-perfused areas (exceeding plasma)
Linezolid Metabolism
Nonenzymatic. Does not utilize CYP
Oxidation of morpholone ring creates 2 inactive metabolites of linezolid
Linezolid Excretion
30% unchanged in urine
65% nonrenal excretion
Use of these drugs & Linezolid is not recommended:
Pseudoephedrine
Vasopressors
Dopaminergics
MAOi’s
Linezolid Considerations:
-Bone Marrow suppression
-Can be used in children
-Serotonin Syndrome

First Line VRE
Greater cure rates of MRSA than Vanco
What foods should be avoided when taking Linezolid?
Tyramine rich foods: aged cheese, wine, beer, chocolate, caffeine
Is Linezolid bactericidal?
No, bacteriostatic
How is Linezolid similar to Clindamycin & Macrolides?
They all bind to 50s ribosomal subunit

TCA’s
Methadone
Ondansetron
Meperidine
