define acute otitis media
inflammation of the middle ear cleft with rapid onset and infectvei origin, assoicated with a middle ear effusion
types of acute otitis media 3
resistant - persistence beyond 3-5 days with ABx
persistent- persistence or recurrence within 6 days of finishing ABx
Recurrent >3 episodes within 6 months or 4-6 episodes within 12 months
investigations for otitis media 3
no initial tests
2nd line - pneumatic otoscopy, tympanometry
symptoms of otitits media WITH effusion
asymptomatic
fulness
popping sensation
decreased hearing or ringing on afffected side
management of otititis media 1
CONSERVATIVE
when would ABx be considered in acute otitis media
only if they fail to improve after 2-3 days of observation
and to all with irregular illness or high risk children
OR UNDER AGE OF 2
ABx in acute otitis media
amox 1st line
augmentin in beta-lactamase resistant
acute otitis media managemnt for reccurrent disease
adenoidectomy but evidence poor
