Universal recommendations for all patients
Balanced diet Limit caffeine to 1-2 servings a day Smoking cessation Avoidance of excessive alcohol intake Regular weight-bearing exercise and muscle-strengthening exercise Fall prevention
Calcium recommended dosing
Adults 50+ should get 1200mg/day, preferably from diet but supplement if necessary
Calcium risks
CV risk, kidney stones, bloating, constipation, acute distress
Calcium drug interactions
Acid-reducing agents, levothyroxine, quinolone, TTC ABX, bisphosphonates
Acid-dependent calcium formulations
Calcium carbonate (take w/ food)
Acid-independent calcium formulations
Calcium citrate (take w/ or w/o food)
Vitamin D recommendations
Adults 50+: 1000 IU/day
Safe upper limit is 4000 IU/day
Optimal level of vit. D
≥30 ng/ml
Deficient vit. D dosing
5000 IU of D3 daily for 8-12 weeks, then 1000-2000 IU QD
Alt: 50000 IU of D3 daily for 8-12 weeks, then 1000-2000 IU QD
