What do you use for OP assessment?
T-score from a DEXA scan
WHO definition of osteoporosis based on BMD: normal
T-score of -1.0 or higher
WHO definition of osteoporosis based on BMD: osteopenia
T-score between -1.0 and -2.5
WHO definition of osteoporosis based on BMD: osteoporosis
T-score -2.5 or lower
WHO definition of osteoporosis based on BMD: severe or established OP
T-score -2.5 or lower with one or more fractures
WHO FRAX algorithm
Used in patients with low bone mass (osteopenia) between the ages of 40-90; doesn’t take into account fall risk
AACE/ACE diagnostic criteria
T-score -2.5 or lower in lumbar spine, femoral, neck, or radius
Low-trauma spine or hip fracture regardless of BMD
Osteopenia AND fragility fracture of proximal humerus, pelvis, or distal forearm
Osteopenia/low bone mass and high FRAX probability (10-year probability of hip fracture ≥3% or 10-year probability of a major OP-related fracture ≥20%)
