Etiology
OS: mechanical
RA: autoimmune, Type III
Joint finding
OS: osteophyte, ivory-bone, DIP (Hebereden) and PIP (Bouchard), no MCP
RA: pannus in MCP/PIP, no DIP, subQ RA nodule (fibrinoid necrosis) ulnar deviation of fingers. subluxation, Baker’s cyst
Predisposing factors
OS: age, obesity, joint deformity
RA: Female > male, 80% with RA (anti IgG), anti cyclic citrullinate peptide Ab, HLA-DR4
Presentation
OS: pain in weight bearing point, improve with rest, bow legged
RA: morning stiffness, improve with use, systemic symptoms (fever, fatigue, pleuritis, pericarditis)
Treatment
OS: NSAIDs, intra-articular glucocorticoid
RA: NSAIDs, glucocorticoid, disease modifying agents (MTX, sulfasalazine, TNF inhibitors)
