Arachidonic acid Flashcards
Lipoxygenase pathway
Leukotrienes
LTC4, D4, and E4
Bronchoconstriction, vasoconstriction, smooth muscle constriction, and increased vascular permability
LTB4
neutrophil chemotatic agent
PGI2
inhibits platelet aggregation
promotes vasodilation
Platelet gathering inhibitor
Aspirin
Both COX1/2, acetylate
Increase bleeding time, but no effect of PT/PTT
Low dose (<300): inhibit platelet aggregation Mid dose (300-2400): anti fever and analgesic High dose (2400-4000): anti inflammatory
Toxicity: tinnitus
Stimulates hyperventilation and resp alkalosis
NSAIDS
Both COX1/2
Renal ischemia
(PG vasodilates afferent arteriole)
COX2 inhibitor
Spares platelet function as THA2 production is dependent on COX2
Use: RA, osteoarthritis, pts with gastritis or ulcers
Toxicity: increased risk of thrombosis, sulfa allergy
Acetaminophen
Reversibly inhibit COX, in CNS
Inactivated peripherally
USe: anti pyretic, analgesic, but not anti inflam
Bisphosphonate
Alendronates or other dronates
Pyrophosphate analog, which binds hydroxyapatide in bone
Inhibiting osteoclast activity
USE: osteoporosis, hypercalcemia, Paget’s disease of bone
Toxicity: corrosive esophagitis, osteonecrosis of jaw
