OSPE Aspergillosis Flashcards
case
-immunocompromised patient
- persistent cough, chest pain, and shortness of breath.
-Imaging reveals multiple nodules in the lungs.
-shows septate
hyphae with acute-angle branching.
-recent chemotherapy for leukemia.
direct microscopy
colony: A wet preparation stained with lactophenol cotton blue demonstrates septate hyphae and conidiophores
Specimen
Exudates, bronchial washings and bronchoalveolar lavage
stain
wet mount
preparation in 10% potassium hydroxide
Tissue sections
stain: periodic acid Schiff
dichotomous branching at an acute angle
Detection of galactomannan
in serum or bronchoalveolar lavage fluid is a marker for the diagnosis of invasive aspergillosis
Serology:
Precipitating antibodies to aspergillus antigens can be demonstrated by counter-current immunoelectrophoresis, immunodiffusion and ELISA.
other
PCR and nucleic acid sequence-based
Skin tests
Dual immediate (after 15 minutes) and Arthus type (after 4-6 hours) skin test reactions which develop after the intradermal injection of Aspergillus spp. antigens
- diagnosis of allergic broncho-pulmonary aspergillosis.
culture
sabouraud agar medium
fumigatus:smoky green,velvety
flavus:yellow-green velvety
niger:black,cottony
