what can cause optic neuritis
MS
antibody mediated
non infectious (autoimmune disease)
infection
post infectious (EBV)
presentation of optic neuritis
post occular pain
flashes (worse with eye movement)
visual field loss and decreased acuity
reduced colour/contrast vision
what is the natural history of optic neuritis
worsens over hours to days
will start to improve within 2 weeks with or without treatment
what investigation can be done for optic neuritis
MRI with gadolinium contrast
shows enhanced optic nerve
may see MS plaques
management of optic neuritis
IV methylprednisolone with prednisolone taper
can reduce risk of MS and is effective within MS patients
