Placental Abruption?

What is it?

A

Is the cause of antepartum haemorrhage

Vaginal bleeding <24 wks gestation

Remembered as PVC2:

  • P - Placental abruption
  • P - Placenta praevia
  • V - Vasa praevia ( babies’ blood vessels cross or run near the internal opening of the uterus. These vessels are at risk of rupture when the supporting membranes rupture)
  • V - Vaginal infection
  • C - Cancer of the cervix
  • C - Cervicitis

Risk factors?

A

Remember as OH PIPS:

  • O - over distended uterus
  • H - Hypertension
  • P - Eclampsia
  • I - Intra-uterine growth restriction
  • P - Past history of placental abruption
  • S - Smoking history

Symptoms?

Investigations?

A

Symptoms:

  • Vaginal Bleeding
  • Severe Abdo pain
  • Systemic Shock
  • Wooden uterus on palpation

Investigations:

  • CTG - Cardiotocography
  • BLood tests: FBC, U&E, group & save
  • Radiology - USS for placenta praevia

Treatment?

Complications?

A

Medical:

  • Emergency treatment: admission, crossmatch and
  • blood transfusion
  • Consider delivery
  • Give mother steroids to encourage lung development

Complications:

  • Fetal death
  • intra-uterine growth restrictions

Maternal DADS:

  • D - Death
  • A - Acute kidney injury
  • D - Disseminated intravascular coagulation/multi-organ failure
  • S - Shock
