STI’s Summary?

Chlamydia?

A

Organism:

  • Chlamydia Trachomatis

Symptoms:

  • Asymptomatic
  • Females-vaginal discharge, inter-menstrual /post coital bleeding, cervicitis
  • Males - urethritis, dysuria
  • Most commom cause of PID

Investigations:

  • NAAT - Nucleic acid amplification test
  • From cervical swab, urine sample

Treatment:

  • Doxycycline (7 days)
  • Azithromycin (single dose)

Trichomoniasis?

A

Organism:

  • Trichomonas vaginalis

Symptoms:

  • Asymptomatic
  • Females: vaginal discharge (green and offensive), vulvovaginitis, ‘strawberry cervix’, superficial dyspareunia, pH>4.5
  • Males: urethritis

Investigations:

  • Wet mount microscopy, can see organisms swimming

Treatment:

  • Metronidazole

Gonorrhoea?

A

Organism:

  • Neiseeia gonorrhoeae

Symptoms:

  • Females: generally asymtomatic, vaginal discharge, cervicitis
  • Males: Urethritis

Investigations:

  • Endocervical swabs

Treatment:

  • Azithromycin
  • IM ceftriaxone

Genital Warts (condylomata accuminata)

A

Organism:

  • Human Papillomavirus

Symptoms:

  • Papilliform or flat warts
  • May be pigmented
  • May bleed
  • May itch

Investigations:

  • Clinical presentation

Treatment:

  • !st line: topical podphyllum or cryotherapy
  • 2nd line: imiquimod cream

Organism:

  • Hepes Simplex Virus (HSV 1 &2)

Symptoms:

  • Painful ulcerated lesions
  • Dysuria
  • Lymphadonopathy

Investigations:

  • Clinical presentation/viral swab

Treatment:

  • Acyclovir

  • Primary syphillis-chancre
  • Secondary syphillis-rash
  • Tertiary syphillis-cardiac and neurological involvement (gummas)

  • Penicillin
