how is OCD classified?
presence of either obsessions or compulsions
what is an obsession?
unwanted intrusive thought, image or urge that repeatedly enters the person’s mind
what are compulsions?
repetitive behaviours or mental acts that the person feels driven to perform
what are the risk factors for OCD?
- family history
- age - peak incidence is between 10-20 years
- pregnancy/postnatal period
- history of abuse, bullying, neglect
what is the management for an OCD patient with mild functional impairment?
- CBT and exposure and response prevention (ERP)
- SSRI
what is the management for an OCD patient with moderate functional impairment?
- SSRI or more intensive CBT
- consider clomipramine
what is the management for an OCD patient with severe functional impairment?
- refer to secondary care mental health team
- SSRI + CBT
when is fluoxetine used in the management of OCD?
body dysmorphic disorder
what is ERP?
exposure and response prevention
exposing patient to an aniexty provoking situation and then stopping them engaging in their usual safety behaviour
how long does it take for an SSRI to have an impact in OCD?
12 weeks
