0.4-1.0 mmol/L
what organ excrets lithium?
kidney
what are the adverse effects of lithium?
- nausea/vomiting, diarrhoea
- fine tremor
- nephrotoxicity
- thyroid enlargement
- weight gain
- idiopathic intracranial hypertension
- leucocytosis
- hyperparathryoidism –> hypercalcaemia
what ECG changes can be seen with a patient on lithium?
T wave flattening/inversion
when should a sample for lithium levels be taken?
12 hours post-dose
when are lithium levels checked?
- beginning = weekly
- after each dose change
- stable = 3 months
when are thyroid and renal functions checked in a patient on lithium?
every 6 months
what concentration leads to lithium toxicity?
> 1.5 mmol/L
what can precipitate lithium toxicity?
- dehydration
- renal failure
- drugs
what drugs can precipitate lithium toxicity?
- thiazides
- ACEi/angiotensin II receptor blockers
- NSAIDs
- metronidazole
what are the features of lithium toxicity?
- coarse tremor
- hyperreflexia
- acute confusion
- polyuria
- seizure
- coma
is fine tremor a sign of lithium toxicity?
no it is seen in therapeutic levels
how is lithium toxicity managed?
- volume resuscitation with normal saline
- severe = haemodialysis
- sodium bicarbonate
