- Examine for perceptual deficits
Stroke Red Flag: Perceptual Deficits
- May result in neglect, body scheme impairment, Pusher’s syndrome which put patients at a high risk for falls and injury.
Examination: Behavior: Left Hemisphere
- Slow
- Cautious
- Hesitant
- Aware of impairments which may cause frustration
Examination: Behavior: Right Hemisphere
- Impulsive
- Quick
- Indifferent
- Often exhibiting poor judgement and safety
- Over estimate ability
- Under estimate impairment
Examination: Sensory Changes
- Examine for sensory changes on contralateral side of the body considering spinal tracts, dermatomes, and respective peripheral nerves.
Examination: Motor Changes
- Examine for tone, primitive reflexes, and synergies
Examination: Gait Abnormalities: Hip
- Trendlenberg: Weak abductors
- Scissoring Gait: Spasticity to the Adductors
- Insufficient pelvic rotation during gait
Examination: Gait Abnormalities: Hip Flexor Deficits
- Circumduction During Swing
- External Rotation with adduction
- BACKWARD LEANING OF TRUNK
- Exaggerated flexion synergy
Examination: Gait Abnormalities: Knee Extensor Deficits/Weakness to knee extensors/knee extension
- Knees flexed during stance, OR
- HYPEREXTENSION OF KNEE/COMPENSATORY LOCKING OF KNEE IN HYPEREXTENSION.
Examination: Gait Abnormalities: Knee: Quadriceps spasticity
- HYPEREXTENSION OF KNEE
Examination: Gait Abnormalities: Ankle: Foot Drop
- Weakness of dorsiflexors/dorsiflexion
Examination: Gait Abnormalities: Ankle: Equinus Gait
- Spasticity to Plantarflexors
- Heel does not contact ground
Examination: Gait Abnormalities: Other
- Step length decrease
- Cadence decrease
