Dosage - Adult

1st dose: 0.4 mg IM/IV - q 3mins
2nd dose: 0.4 mg IM/IV
3rd dose: 0.8 mg IM/IV
4th dose : 2 mg IM/IV - (IV preferred)

palliative pts - titrate 0.1mg IM/IV/min - for reversal of resp depression w/o interfering w/ pain control

*higher doses for peds - they are unlikely to experience withdrawal

Pharmacodynamics

- competitively antagonizes opioids bound to receptor in the CNS
opiod receptors mu/kappa/gamma - strongest binding to mu receptor

peak - 5-15mins
half-life - 1-3hrs
duration - 30-45mins
Warnings // Precautions

- maybe heat sensitive - may lose potency post 4wks @40C and will require use of higher doses
ADVERSE EFFECTs

- SUDDEN reversal => combativeness
- S&S of withdrawl
- Hypotension, hypertension
- Nausea, vomiting, sweating,
- Tachycardia
