miscellaneous

ADVERSE EFFECTs

dexamethasone

A
  • wound healing impairment
  • local : postinjection flare (intra-artivular use)
  • hematologic & oncologic : Kaposi sarcoma, petechial, tumor lysis syndrome
  • genitourinary : defective (increased/decreased) spermatogenesis
17
Q

Warning // Precautions

dexamethasone

A
  • may cause hypercortisolism, particularly in younger children/when used for long periods of time at higher doses
  • should generally not be used for adrenal insufficiency - lacks mineralcorticoid activity
  • used w/ caution in pts w/ CHF or HTN - associated w/ fluid retention and electrolyte disturbance
  • corticosteroids have been associated w/ myocardial rupture when used in AMI
  • crosses the placenta - some studies found an association betwen use in 1st trimester and decreased birth weights and oral clefts
18
Q

DRUG INTERACTIONs

dexamethasone

A
  • may enhance adverse/toxic effects of NSAIDs and salicylates (i.e. GI ulceration and bleeding)
  • may reduce serum concentration of salicylates
  • may decrease serum concentraion of phenytoin
  • may enhance anticoagulant properties of warfarin
