Why do we use the MUST score
Screening tool to detect patients at risk of malnutrition
Step 1 - BMI, scoring
-if unable to get measurements, what else could you do?
BMI
0 - 20+ (if obese, 30+)
1 - 18.5-20
2 - U18.5
Self reported values
Height from ulnar length
BMI from mid upper arm circumference
If fluid retaining, consider and estimate
Step 2 - unplanned weight loss in past 3-6 months, scoring
0 - U5%
1 - 5-10%
2 - 10%+
Step 3 - acute illness AND no intake for 5+ days, scoring
0 - no
2 - yes
How would you manage a
- low risk
- medium risk
- high risk
Low risk - 0
-repeat screen
Medium risk - 1
- document intake for 3days
- adequate => same as low risk
- inadequate => local policy to increase and improve intake
High risk - 2
-dietician referral => increase and improve intake
How could you support nutrition intake?
Oral intake
- fortify with milk powders, custard, creams
- full fat products
- offer regular snacks
Oral supplements
-fortisip, fortijuice
Enteral feeding
- unable to tolerate oral intake
- NG - short term feeding
- RIG, PEG - long term feeding
- TPN - non functioning GI with obstruction
