Primary hyperPTH - most common in non hospitalised
Malignancy - most common in hospitalised
- bone mets
- myeloma
- ectopic PTH from squamous lung
Milk alkali - excess intake of Ca and absorbable alkali

Other possible causes
A
Sarcoidosis => granuloma formation
Thyrotoxicosis => increased bone breakdown
Acromegaly
Paget’s bone
Addisons/dehydration => decreased excretion
VitD intox => too much Ca absorbed via kidney and gut, bone resorption

Management
A
Initial - rehydration with saline
Followed by bisphosphonates
-max effect at 1wk
Steroids in sarcoid
Calcitonin acts faster than bisphosphonates
