Bamboo spine
Due to Ankylosing spondylitis
MS inflammatory dz due to spondylitis and sacroilitis
90% HLAB27 positive
Young men in 2-3 decade, some get hip and shoulder arthritis
Aoritc inflamm, lung fibrosis, uveitis
Carpometacarpal squaring
Of thumb due to OA
Bones on bone joints with osteophyte and crepitus
Dactylitis
Asymm inflam oligoarthritides
Sausage digits
Spondyloarthropathy
DIP arthritis, spine involvement, enthesistis
Psoriasis, inflamm bowel, AS, and reactive arthritis
Carpal tunnel
Phalens test - compression of wrists together produces paresthesia in median nerve
Tinnel - percussion of median nerve produces parestheisa in median nerve
Finkelstein
Pain eleicted by flexing thumb into palm…bending wrist in ulnar
DeQuervians tenosynovisitc - look for tenderness at snuffbox
Draw sign
Flex to 90 dg and pull anteriorly
McMurray
Varus and valgus stress
Clicking means meniscal injury
Pes anserin bursitis
Pain with exercise and or stair climing inferiro to knee
Tenderness to percussion over bursa is confirmatory
Painless ext subq nodules
RA
May have other manifestations of RA too
Could get lung firboriss or empyema mimicking pleural effusion
Septic arthritis
Hemoatongeous seeding to joint space and large monoarthritis
Adhesive capsulitis
Reduced passive AND active ROM with pain
Aply scratch test looks ofr ROM and positive tocuhdown sign expected
Bicipital tendonities
Pain with lifitng, pulling or overhead movements…confirm with palpaton of bicipital tendon
Drop ar mtest
Raise are to 90 deg of abduction and lower slowly…if sudden drop then rotator cuff
Spine defomriaity
Scoilios - lateral
Lordosis - back
Kyphoiss- forward
SLE
2/3 have frank arthritis
Most polyarticular and symmetric but NOT deforming
Hypercoag syndrome related to antiphospholipid
