Steps of the Exam
A
- introductions
- patient/caregiver interview - start judging speech & gather primary concerns
- physical exam - wash hands, gloves, good visual inspection, don’t apologize

Salient features to look at
A
- strength
- speed of movements (slowness common in MSD)
- range of movement (influence on prosody & articulation)
- Steadiness (tremors, involuntary movements)
- Tone (can be excessive or reduced)
- Accuracy (perceived in articulation & prosody)

What might facial asymmetry suggest?
A
- neuromuscular impairment
- craniofacial anomaly

What might flaccidity suggest?
A
- atrophy
- paresis/paralysis (CN VII)

What might spasticity suggest?
A
- hypertonic muscle activity
- damage to UMN fibres of pyramidal tract

What might drooping lip on one or both sides suggest?
A
- Flaccidity (ipsi CN VII)
- Spasticity (contra pyramidal)

What might drooling suggest?
A
- swallowing difficulties
- some medications
- nervousness
- lack of awareness

What might abnormal/adventitious movements suggest?
A
- tics (localized spasmodic movements)
- chorea, dyskinesias, dystonias

What might fasciulations suggest?
A
- can be benign
- may be loss of innervation of LMN
- if you see these, do a second look & take your time

What might myoclonus suggest?
A
- Damage to CNS

What might presence of path. reflexes suggest?
A
- CNS pathology (esp frontal lobes or subcortical regions)
- use caution interpreting these
