Dementia
progressive mental disorder characterized decline of mental function, impairment of memory, judgement, impulse
Advance Directives
document communicates patients specific wishes for end-of-life care should patient become unable to do so
Durable Power of Attorney for Health Care
document which patient designate someone to make health care decisions for them if they are unable to do so themselves
