Scapular Abduction (protraction) and Upward Rotation Grades 3-5
Short sitting- arm flexed to 130* and protracted
Scapular Abduction (protraction) and Upward Rotation Grades 0-2
Short sitting- arm forward flexed above 90* and supported by therapist
Scapular Elevation Grades 3-5
Short sitting- hands relaxed in lap “shrug”
Scapular Elevation Grades 0-2
Prone- head turned away from test side “shrug”
Scapular Adduction (Retraction) Grades 0-5
Prone- shoulder at edge of table, shoulder abducted to 90, elbow flexed to 90, with forearm and hand off table
“lift your elbow toward the ceiling”
Scapular Depression and Adduction (Retraction) Grades 0-5
Prone- test arm over head at 145* with forearm in midposition and thumb pointed toward ceiling, head turned toward test side.
Scapular Adduction (Retraction) and Downward Rotation Grades 3-5
Prone- shoulder is internally rotated and arm is adducted across the back with elbow flexed and hand resting on back
“lift your hand from your back”
Scapular Adduction (Retraction) and Downward Rotation Grades 0-2
Short sitting- shoulder internally rotated and arm extended/adducted behind back
“try to move your hand away from your back”
Shoulder Extension, Adduction, Medial Rotation (Latissimus Dorsi) Grades 0-5
Prone with head turned to test side, arms at sides, and shoulder internally rotated
Shoulder Flexion Grades 3-5
Short sitting- with arms at sides, elbow slightly flexed, forearm pronated
Shoulder Flexion Grades 0-2
Side lying- Test side up, arm flexed to 90*
Shoulder Extension Grades 0-5
Prone- with arms at sides and shoulder internally rotated (palm up)
Shoulder Abduction Grades 3-5
Short sitting- arm at side with elbow slightly flexed

Supine- arm at side supported on table in neutral rotation (thumb pointed outward)
Shoulder Abduction Grades 0-1
Supine- with arm at side and elbow slightly flexed
Shoulder Horizontal Abduction Grades 3-5
Prone- shoulder abducted to 90* and forearm off edge of table with elbow straight
Shoulder Horizontal Abduction Grades 0-2
Short Sitting
Shoulder Horizontal Adduction Grades 3-5
Whole: supine- shoulder abducted to 90, elbow flexed to 90
Clavicular: supine- shoulder abducted to 60* with elbow flexed
Sternal: supine- shoulder abducted to 120* with elbow flexed
Shoulder Horizontal Adduction Grades 0-2
Supine- arm supported in 90* of abduction with elbow flexed to 90*
Shoulder External Rotation Grades 3-5
Short sitting- elbow flexed to 90* and forearm in neutral position, perpendicular with patient’s trunk
Shoulder External Rotation Grades 0-2
Short sitting- elbow flexed to 90*, forearm in neutral rotation with hand facing forward, supported on the table with friction minimized by therapist
Shoulder Internal Rotation Grades 3-5
Short sitting- with elbow flexed to 90*, forearm in neutral perpendicular to trunk
Shoulder Internal Rotation Grades 0-2
Short sitting or sitting at table- elbow flexed and forearm in neutral rotation
Elbow Flexion Grades 3-5
Short sitting- arms at sides (biceps: forearm in supination, Brachialis: forearm in pronation, Brachioradialis: forearm in neutral)
