Hip Flexion (Grade 4-5)
“sit tall and hold your thigh up, don’t let me push it down”
Resistance: over distal thigh just proximal to knee joint in a downward direction
Hip Flexion (Grade 2)
“bring your knee up toward your chest”
One hand: cradle test limb with hand supporting under flexed knee
Other hand: maintain trunk alignment at hip
Hip Flexion (Grade 0-1)
“try to bring your knee up to your nose”
One hand: support test limb under calf with hand behind knee
Other hand: palpate Iliopsoas just distal to inguinal ligament on medial side of sartorius
Hip Flexion, Abduction, and External Rotation with Knee Flexion (Grade 4-5)
“hold it, don’t let me move your leg or straighten your knee”
One hand: on lateral side of knee (up and out direction)
Other hand: over medial-anterior surface of distal leg (down and in direction)

“slide your heel up to your knee”
One hand: support limb at calf
Other hand: palpate sartorius

One hand: cradle test limb under calf with hand supporting limb behind knee
Other hand: palpate sartorius on medial side of thigh where muscle crosses femur
Hip Extension (ALL) (Grade 5)
“lift your leg up off table as high as you can without bending your knee, hold it”
One hand: stabilize pelvis at PSIS
Resistance: on posterior leg just above the ankle in a downward direction

One hand: stabilize pelvis at PSIS
Resistance: on posterior leg just above the knee in a downward direction
Hip Extension (ALL) (Grade 2)
“bring your leg back toward me, keep your knee straight”
One hand: support test limb just below the knee, cradling the leg
Other hand: over the pelvic crest to maintain pelvic and hip alignment
Hip Extension (ALL) (Grade 0-1)
“try to lift your leg from the table/squeeze your buttocks together”
Palpate: hamstrings and gluteus maximus
Hip Abduction (Grade 5)
“lift your leg up and back, hold it, don’t let me push it down”
Stabilization: at pelvic crest to maintain alignment
Resistance: at ankle in a downward direction
Hip Abduction (Grade 4)
“lift your leg up and back, hold it, don’t let me push it down”
Stabilization: at pelvic crest to maintain alignment
Resistance: at lateral knee in a downward direction
Hip Abduction (Grade 0-2)
“try to bring your leg out to the side”
One hand: support limb under ankle just above malleoli
Other hand: palpate gluteus medius on lateral aspect of hip just above the greater trochanter

Stabilization: on the crest on the ilium for pelvic alignment
Resistance: on lateral surface of thigh just above knee in downward direction
Hip Abduction from a Flexed Position (Grade 2)
“bring your leg out to the side”
One hand: supports limb under ankle
Other hand: palpates TFL on proximal anterolateral thigh
Hip Abduction from a Flexed Position (Grade 0-1)
“try to move your leg out to the side”
One hand: palpates TFL on lateral aspect of knee
Other hand: palpates TFL on anterolateral thigh
Hip Adduction (Grade 4-5)
“lift your bottom leg up to your top one, hold it, don’t let me push it down”
One hand: support uppermost limb in 25* of abduction with forearm, arm supporting the limb on the medial surface of knee
Other hand: on medial surface of the distal femur of the lower limb just proximal to knee joint in a downward direction
Hip Adduction (Grade 0-2)
“bring your leg in toward the other one”
One hand: supports ankle and elevates it slightly from table
Other hand: palpate adductor mass on inner aspect of the proximal thigh
Hip External Rotation (Grade 4-5)
“don’t let me turn your leg out”
Resistance hand: on medial aspect of ankle just above malleolus (in an outward direction)
Other hand: contoured over lateral aspect of distal thigh just above the knee (in a medially directed force)
Hip External Rotation (Grade 2-3)
Grade 2: completes full ROM without resistance
Grade 3: completes full ROM with mild to no resistance
Hip External Rotation (Grade 0-1)
“try to roll your leg out”
cannot palpate ER muscles except gluteus maximus
Hip Internal Rotation (Grade 4-5)
“don’t let me turn your leg in”
Resistance hand: on lateral surface of ankle just above malleolus (in an inward directed force)
Other hand: contoured over medial surface of distal thigh just above knee (in an outward directed force)
Hip Internal Rotation (Grade 2-3)
Grade 2: completes full ROM without resistance
Grade 3: completes full ROM with mild to no resistance
Hip Internal Rotation (Grade 0-1)
“try to roll your leg in”
One hand: palpate gluteus medius over posterolateral surface of hip above the greater trochanter
Other hand: palpate TFL on anterolateral surface of hip below ASIS
