Calcium Chloride
Description
Increases myocardial contractile force and increases ventricular automaticity
Calcium Chloride
Indications
Hyperkalemia, hypocalcemia, hypermagnesemia, calcium channel blocker toxicity
Calcium Chloride
Contraindication
Ventricular fibrillation, hypercalcemia, possible digitalis toxicity
Calcium Chloride
Precautions
May precipitate toxicity in patients taking digoxin. Ensure IV line in large vein and flushed before and after calcium
Calcium Chloride

Osmitrol
Another Name
Mannitol
Mannitol (Osmitrol)
Class
Osmotic Diuretic
Mannitol (Osmitrol)
Description
Osmotic diuretic that draws water into the intravascular space through hypertonic effects, causes diuresis
Mannitol (Osmitrol)
Indications
Cerebral edema
Mannitol (Osmitrol)
Contraindications
Hypersensitivity, pulmonary edema, severe dehydration
Mannitol (Osmitrol)
Precautions
Must give slow with filter tubing
Mannitol (Osmitrol)
Dose/Route
1.5-2 g/kg slow IV
Pedi 0.25-0.5 g/kg over 60 min
Phenytoin
Another Name
Dilantin
Dilantin
Another Name
Phenytoin
Phenytoin (Dilantin)
Class
Anticonvulsant
Phenytoin (Dilantin)
Description
Derivative related to phenobarbital that reduces spread of electrical discharges in motor cortex and inhibits seizures. Has antidysrhythmic properties that counteract effects of digitalis
Phenytoin (Dilantin)
Indications
Seizures, status epilepticus, cardiac dysrhythmias secondary to digitalis toxicity
Phenytoin (Dilantin)
Contraindications
Seizures due to hypoglycemia, sinus bradycardia, heart block, Adam-Stokes syndrome
Phenytoin (Dilantin)
Precautions
Hepatic or renal impairment, alcoholism, cardiogenic shock, elderly, bradycardia, respiratory depression
Phenytoin (Dilantin)
Dose/Route
Seizures/status epilepticus: 10-15 mg/ kg slow IV. No more then 50 mg/min
Dysrhythmias: 100 mg slow IV (over 5 min) to maximum of 1000 mg
Phenytoin (Dilantin)
Pedi Dose/Route
Seizures/status epilepticus: 8-10mg/kg slow IV
Dysrhythmias: 3-5 mg/kg slow IV
Tetracaine
Class
Opthalmic, anathetics
Tetracaine
Description
Topical application to the eye, local anesthetics penetrate to sensory nerve endings in corneal tissue. Blocks both initiation and conduction of nerve impulses by decreasing neuronal membranes permeability to sodium ions. Reversibly stabilizes the membrane and inhibits depolarization, resulting in failure of a propagated action potential and subsequent conduction block
Tetracaine
Indications
Eye injuries or foreign body
Tetracaine
Contraindication
Hypersensitivity
Tetracaine
Precautions
Hyperthyroid disease, cardiac disease
Tetracaine
Dose/Route
1-2 drops per eye
Timoptic
Class
Beta blocker
Timoptic
Description
Non-selective beta-adrenergic receptor blocking agent
Timoptic
Indications
HTM, MI elevated ocular pressure
Timoptic
Contraindications
Asthma, severe COPD, sinus brady, 2nd or 3rd degree AV block, cardiogenic shock
Timoptic
Precautions
Readily absorbed
Timoptic
Dose/Route
Ophthalmic: 1 drop per eye BID
Sodium Bicarbonate
Anther Name
NaHCO3
NaHCO3
Another Name
Sodium Bicarbonate
Sodium Bicarbonate (NaHCO3) Description
Provides vascular bicarbonate to assist buffer system in reducing the effects of metabolic acidosis and treatment of some overdoses
Sodium Bicarbonate (NaHCO3) Indications
Tricyclic antidepressant and barbiturate overdose, refractory acidosis and hyperkalemia
Sodium Bicarbonate (NaHCO3) Contraindications
None where used for severe hypoxia or late cardiac arrest
Sodium Bicarbonate (NaHCO3) Precaution
May cause alkalosis if given in large quantities. May deactivate vasopressors and may precipitate with calcium chloride
Sodium Bicarbonate (NaHCO3) Dose/Route
1 mEq/kg IV, hen 0.5 mEq/kg/ 10 min
Pedi: same, may also be given IO
