What is MSCC?
Compression of the spinal cord +/- cauda equina as a result of malignant disease
What is the mechanism of MSCC?
Collapse/compression of vertebral body with metastases
What happens with cord compression?
Oedema
Venous congestion
Demyelination
What can happen with prolonged compression?
Vascular injury
Cord necrosis
What are the features of MSCC?
Back pain
- prolonged
- worse with straight leg raise
- worse with lying down
Limb weakness
Sensory deficits
Bladder/bowel dysfunction
How is MSCC managed?
Dexamethasone 16mg
Surgery
Radiotherapy
When should you not give dexamethasone in MSCC?
?Lymphoma
How soon should an MRI be offered in pain suggesting MSCC?
Within 1 week
How soon should an MRI be offered with signs of MSCC?
Within 24 hours
How can MSCC be prevented?
Patient education if high risk
