mesenteric ischaemia Flashcards
what
lack of blood flow through mesenteric vessels resulting in intestinal ischaemia
Blood supply to abdo organs
coeliac artery
SMA
IMA
chronic mesenteric ischaemia
result of narrowing of mesenteric blood vessels by atherosclerosis
results in intermittent abdo pain when supply cant keep up with demand
chronic mesenteric ischaemia classic triad
- central colicky abdo pain after eating
- weight loss due to food avoidance (pain)
- abdo bruit on auscultation
chronic mesenteric ischaemia risk factors
- age
- FHx
- smoking
- diabetes
- htn
- raised cholesterol
chronic mesenteric ischaemia diagnosis
CT angiography
chronic mesenteric ischaemia Mx
- reduce modifiable risk factors
- 2ry prevention: statins, antiplatelet
- revascularisation
chronic mesenteric ischaemia Mx - revascularisation
- endovascular procedures first line
- open surgery
acute mesenteric ischeamia - typically caused by
rapid blockage in blood flow though SMA - usually thrombus
AF is key risk factor
acute mesenteric ischeamia - presentation
acute, non-specific abdo pain
pain disproportionaet to exam findings
acute mesenteric ischeamia - diagnostic test
contrast CT
also metabolic acidosis and raised lactate
acute mesenteric ischeamia - Mx
surgery
- remove necrotic bowel
- remove or bypass thrombus
