Macrolides
Erythromycin, clarithromycin, azithromycin
Macrolide Spectrum
Good: atypicals, H flu, Moraxella, H pylori, Mycobacterium
Mod: strept
Clarithromycin Dose
PO: 250-500 mg bid or 1 g daily
Clarithromycin Renal
0-30: half dose
Azithromycin Dose
IV: 500 mg q24h, no renal
PO: 250-1000 mg daily
Erythromycin Dose
IV: 500-1000 mg q6h, max/day: 4 g. no renal
PO: 250-600 q6-12h
Folate Antagonists
TMX/SMX, dapsone, sulfadiazine
Folate Antagonist Spectrum
Good: MSSA (some MSRA), H flu, Stenotophmonas, PJP,
Mod: enteric GNRs, S. pneumo, Salmonella, Shigella, Nocardia
TMP/SMX Dose
IV: 8-20 mg TMP/kg/day q6-12h
PO: 1-2 DS tabs q12-24h
TMP/SMX Renal
15-30: half, 0-15: not rec, full q48h
Dapsone Dose
50-300 mg PO daily in divided doses, no renal
Sulfadiazine Dose
2-4 g/day PO in 3-6 divided doses
