Aztreonam Spectrum
Good: pseudomonas, most GNRs
Mod: acinetobacter
Aztreonam Dose
IV/IM: 0.5-2 g q6-12h, max/day: 8 g
Aztreonam Renal
10-30: half; 0-10: fourth
Carbapenems
Imipenem/cilastatin, merropenem, ertapenem, doripenem
Carbapenem Spectrum
Good: MSSA, strept, enteric and ESBL GNRs, Pseudomonas (not erta), Acinetobacter (not erta)
Mod: enterococci (not erta)
Imipenem/cilastatin Dose
IV: 250-1000 mg q6-8h, max/day: 4 g
Imipenem/cilastatin Renal
Based on total daily dose, CrCL and/or body weight
Meropenem Dose
IV: 0.5-2 g q8h, max/day: 6 g
Meropenem Renal
26-50: q12h, 10-25: half q12h, 0-10: half q24h
Ertapenem Dose
IV: 1 g q24h
Ertapenem Renal
0-30: 500 mg q24h
Doripenem Dose
IV: 500 mg q8h
Doripenem Renal
30-50: 250 mg q8h, 11-29: 250 mg q12h
