FQ Interactions
Decr. bioavailability with Ca, Fe, antacids, and multivitamins
Levofloxacin Dosing
IV/PO: 250-750 mg q24h
Levofloxacin Renal
20-49: 750 mg q48h, 0-20: 750 mg x 1 then 500 mg q48h
Ciprofloxacin Dosing
IV: 200-400 mg q8-12h, PO: 500-750 mg
Ciprofloxacin Renal PO
30-50: 500 mg q12h, 5-29: 500 mg q18h
Ciprofloxacin Renal IV
0-30: q18h
Tetracyclines
Tetracycline, doxycycline, minocycline
Tetracycline Spectrum
Good: atypicals, rickettsia, spirochetes (H. pylori), Plasmodium
Mod: staph (MRSA), S. pneumo
Tetracycline Dose
PO: 250-500 mg q6-12h,
Doxycycline/Minocycline Dose
IV: 100-200 mg/day in 1-2 doses
Tigecycline Spectrum
Good: atypical, enterococci (VRE), staph (MRSA), S. pneumo
Mod: most GNRs, anaerobes
Tetracycline Renal
10-50: q12H, 0-10: q24h
