Per 40AC-9ZZ06, Locked Valve Control, when is an IV required?
When Locking and Sealing VALVES, BREAKERS and COMPONENTS as follows:
- Required position and locking device
- Local and Remote indication, verify locking device
- If unable to verify, can remove lock per CRS
- For throttled, Concurrent verification, then 2nd IV
- Lock can’t be defeated without damage and Lock doesn’t damage other components
How often are AUDITS required for Locked Valve/Breaker/Component Alignment Lists?
At least ONCE during every refueling outage
*Seals don’t need to be removed for the checks unless CRS/SM directed
If an existing tag references a SHIFT SUPERVISOR (previous terminology) when is required?
NOTHING needs to be relabeled as this is defined as Equivalent Terminology for SM
For Valves, how do we attach a Tag?
With WIRE CABLE and a SEALING DEVICE should be attached to the cable to prevent Manual Operation

- Obtain SM/CRS approval
- Complete current Change Record
- Remove Locking Seal
- Position Valve, Breaker, Component
- If remotely operated valve is placed in manual override, then notify CR of condition
What LIMITS are imposed when relying on an individual to Close a CIV?
Has to be able to respond to close valve in 60 seconds
- 60 seconds is TOTAL time to close valve per DBM
- No concurrent duties that would delay valve closure beyond 60 seconds
