Lidocaine HCL:class
ventricular anti-arrhythmic
Lidocaine HCL:drug actions
decreases automaticity of ventricular cells;increases ventricular fibrillation threshold
Lidocaine HCL:route
IV/IO, give IV push or consider IV drip if rhythm is converted
Lidocaine HCL:adult dose
VF/pulseless VT:1.5 mg/kgunstable VT:1 - 1.5 mg/kgstable VT:1 mg/kgsalvos or runs of VT or PVCs:1 mg/kgrepeat doses at 0.5 - 0.75 mg/kg;max dose is 3 mg/kg;other doses:pain related to IO, 40 mg IO;PAI pre med 1 mg/kg

1 mg/kg
Lidocaine HCL:drip dose
administer IVPB 1 g/250 ml or 2 g/500 ml D5W;1 mg/min = 15 gtts/min2 mg/min = 30 gtts/min3 mg/min = 45 gtts/min4 mg/min = 60 gtts/min
Lidocaine HCL:indications
ventricular rhythms of irritability:PVCs, usually > 6 min, with AMI;runs of VT;VFprophylaxis:to prevent recurrence of VF or VT after initial defibrillation or cardioversionpremedication:PAI
Lidocaine HCL:precautions
consider reduced dose if > 65 yr/old or with PMHx of liver disease or CHF;beware of any caine allergies prior to administration;remember short half-life - hang a gtt post bolus or be prepared to re-bolus;continually monitor ECG, BP, and LOC;if bradycardia presents with PVCs or runs of VT, TREAT BRADYCARDIA FIRST
Lidocaine HCL:side effects
toxicity:early - drowsy, dizzy, lightheadedlate - hypotension, bradycardia, seizure, change in LOC
Lidocaine HCL:contraindications
ventricular escape beats/rhythms associated with bradycardia or heart blocks
