Amiodarone:class
antiarrhythmic
Amiodarone:adult dose
cariac arrest:-first dose 300 mg IV/IO-second dose, if needed, 150 mg IV/IOstable wide QRS tachycardia:-first dose 150 mg over 10 min, repeat as needed if VT recurs-maintenance infusion of 1 mg/min for the first 6 hrs and 0.5 mg/min for next 18 hrs
Amiodarone:pedi dose
refractory VF, pulseless VT:-5 mg/kg IV/IO, repeated to total dose of 15 mg/kg IV/IO per 24 hour, max single dose of 300 mgperfusing supraventricular and ventricular dysrhythmias:-5 mg/kg in 100 ml NS, slow IV over 30 min, repeated to total dose of 15 mg/kg IV per 24 hour, max single dose of 300 mg
Amiodarone:drug actions
alters movement of sodium, potassium, and calcium through normal channels in all cardiac tissues;alpha and beta blocking properties;increase the refractory period of cardiac tissue;slows repolarization;decreases automaticity
Amiodarone:indications
VF/VT cardiac arrest unresponsive to CPR, defibrilation, and vasopressor;recurrent, hemodynamically unstable VT;option in stable wide QRS tachycardia
Amiodarone:side effects
rapid infusion may lead to hypotension;headache;heart block, bradycardia, CHF;liver and thyroid function abnormalities;dizziness;flushing;abnormal salivation
Amiodarone:interactions
do not administer with other drugs that prolong QT interval, Procainamide;may potentiate bradycardia and hypotension with beta blockers and calcium channel blockers;may increase anticoagulant effects of Warfarin
Amiodarone:contraindications
hypersensitivity to drug or iodine;in patients in cardiogenic shock, second or third degree AV block, bradycardia unless an artificial pacemaker is present;do not administer with other drugs that prolong the QT interval;trauma patients;use cautiously in patients with pulmonary, hepatic, or thyroid diseases
