Where the registered nutritionist collects and documents data pertinent to the client
What is a nutritional diagnosis?
Based on the assessment data collected a nutrition problem may be diagnosed. Causes and contributing factors are identified
What is nutrition intervention?
Problems are addressed to improve signs and symptoms. Activities are given so client can work towards objectives
What is nutrition monitoring and evaluation?
Progress made on goals and outcomes are tracked, adjustments are made accordingly
What does NCP stand for?
The nutrition care process model
What is the central core of NCP?
Relationship between client and professional, individualised and patient centered care
What are the outer rings of NCP?
Environmental factors impacting the clients ability to receive and benefit, professional strengths of professional
