LAs Flashcards
Ester LA
procaine - poor tissue penetration, short half life
Amide LA
lidocaine/lignocaine - metabolised by n-dealkylation in liver - longer half life
Unexpected LAs at high concs (non specific)
morphine and propranolol
Atypical LAs
Benzocaine - no amino group, always unprotonated, only works through hydrophobic pathway
QX314 - always protonated, used experimentally, has to be introduced into cell
Prolonging agent for LA
vasoconstrictor e.g adrenaline
Less vasodilatation
mepivicaine
cardiotoxicity
bupivicaine
can induce paraesthesia
articaine (dentistry)
spinal block (surgery of abdomen/pelvis/leg)
bupivicaine (long duration, less doses needed)
lignocaine
tetracaine
into subarachnoid space
surface anaesthesia (e.g. topical,corneal surgery)
tetracaine, lignocaine
infiltration anaesthesia
most, often with adrenaline, never in peripheries
I.V. regional e.g. limb surgery (use torniquet)
lignocaine, prilocaine
I.V. for neuropathic pain
lignocaine
nerve block (trunk/plexus)
most LAS
