A medical report is part of a patient’s medical record and it contains the patient’s complaints, visits to the hospital, and the treatment prescribed by the treating doctor.
Un informe médico es parte de los antecedentes médicos del paciente, y contienen las quejas del paciente, las consultas que hizo en el hospital, y tratamiento que le recommended médico que le estaba tratando.
I would like to go over the various entries included in your report along with additional information on your insurance carrier and copies of various tests that were taken within the last year. One of the entries in your medical report shows that you were admitted to the emergency room with a gun wound which was the result of being involved in a drive-by shooting incident. Another entry shows that you came to our hospital because you felt excruciating pain in the upper part of your abdomen.

Tests revealed an acute inflamation of the muscle that covers the left side of your thorax. This inflamation was due to the car accident you were in two weeks ago prior to visiting our hospital. In addition to the aforementioned entries, your medical report contains copies of various tests such as blood and urine tests.
Las pruebas y análisis revelaron una inflamación aguda del músculo que cubre el flanco izquierdo del tórax. Esta inflamación se debió a un accidente automovilístico que Ud. tuvo dos semanas antes de venir a nuestro hospital. Some of those copies are written in Spanish and a corresponding translation is also attached. A copy of your insurance policy along with several unpaid medical bills are also included in your medical report. I would also like to remind you that our hospital keeps medical reports for a period of five years. Therefore, if you wish to have our hospital mail any information contained in your medical report to another person, please sign this release form, otherwise we will not be able to release any information to any other hospital or doctor.
