What are the 3 organisms most likely to cause dermatophyte infection?
- Microsporum - WON’T cause tinea unguium
- More common worldwide cause - Epidermophyton - common cause of tinea cruris
- Trichophyton - most common US cause
What diseases are caused by dermaphytes?
Tinea + body part
What are the 2 types of tinea capitis?
Ectothrix - outside –> in destruction
- Does not damage hair bulb, hair will likely grow back
Endothrix - infection starts inside the hair bulb –> out
Which bug usually causes endothrix tinea capitis?
T.tonsurans = US
Microsporum world wide
What patients usually get tinea capitis?
Kids!
Treat tinea capitis
- Shampoo
- PO terbinafine
- Wash everything that could be contaminated!
What are the 3 types of tinea pedis?
Interdigital
Moccasin
Vesiculobullous
How do you get tinea corporis?
Direct body contact
- To humans: wrestlers
- To animals: horses
What is a tinea infection of the nails called?
Onychomycosis
How do you get/treat onychomycosis?
Can get independently or as a progression of other tinea infections (tinea pedis)
Treat with PO always! Need to penetrate the nail
- Terbinafine
If you’re using terbinafine as your first choice anti-fungal in most cases when you need a PO drug, what is your second choice?
Itraconazole
How do you treat most tineas (skin based infections)?
Topical: clotrimazole + miconazole
Add PO if severe
What bug causes tinea versicolor?
Malassezia furfur
What are the symptoms of tinea versicolor?
Hyper/hypopigmentation
Dandruff
Dermatitis
Folliculitis
If you scrape sample tinea versicolor, what do you see?
Spaghetti and meatballs
