What is induction of labour?

A
  • Amniotomy - Labour initiated by use of medication or artificial rupture of amniotic membranes
  • IV oxytocin

What are indications for induction of labour?

A
  • Maternal illness - HTN, DM
  • Complicated pregnancy - prolonged (40-42w), twins
  • Complicated birth - Prelabour rupture of membranes, Previous stillbirth or uterine death
  • Foetal size - growth restrictions, macrosomia
  • Maternal request - exceptional

What are contraindications for induction of labour?

A
  • Placenta praevia
  • Prolapsed umbilical cord
  • Foetal distress
  • Malpresentation
  • Anatomical abnormalities e.g. pelvic tumour

What are relative contraindications for induction of labour?

A
  • Previous caesarean sections

What is the score used for predicting whether induction of labor will be required?

A

Bishop’s score - higher is most likely to achieve a successful induction.

> 7 considered favourable for amniotomy.

<5 means cervical ripening agents need to be used.

What are examples of cervical ripening agents?

A
  • Prostaglandins - misoprostol, pessary/vaginal gel
  • Cervical balloon - good cos less likely to cause foetal distress
  • Oxytocin - beware uterine hypertonicity. CTG monitoring
