What is an imperforate hymen?
Cngenital abnormality where the hymen at entrance of vagina fully formed without opening
Where do the upper vagina, cervix, uterus and fallopian tubes develop from?
Paramesonephric ducts (Mullerian ducts)
What happens to paramesonephric ducts in male
anti-mullerian hormone produced causing them to disappear
When does an imperforate hymen present and how
Should be diagnosed at birth by paediatrician
Girl starts to menstruate, menses sealed in vagina causing cyclical pelvic pain and crmaping without vaginal bleeding
How imperforate hymen diagnosed
Clinical exam, transabrominal US
MRI to assess thickness and therefore surgical choice
Risk of untreated perforate hymen
Secondary endometriosis
How is imperforate hymen treated
thin hymen - vertical incision
Thick hymen - surgical resection
Fic vaginal wall to intoitus = avoids restensosi and complication free
