Physiopath IA sur VG et TA
. Augmentation volume VG + pressions =
1 = dilatation
2 = HVG par augmentation des contraintes pariétales
. Augmentation PAS et diminution PAD =
- hyperpulsatilité avec augmentation P différentielle et diminution PAD
. Diminution PAD aorte entraîné une hypoperfusion coronaire relative
Conséquences IA aiguë (EI)
- OAP
- hypoperfusion coronaire
Étiologies IA :
- DYSTROPHIQUE : IA annulo-ectasiante par dilatation de l’anneau + de l’aorte ascendante (aspect valve piriforme ou en bulle d’oignon, mais morpho normale)
Risque de DISSECTION Ao - Séquelle EI
- BICUSPIDIE : dilatation de l’aorte associée ou sténose (= maladie aortique)
- Mdie inflammatoires (Takayasu, HLA B 27, PR)
- Dérives ergot de seigle, anorexigènes (idem IM)
- Rhumatismale rare
Clinique IA / ECG
- dyspnée
- angor (baisse PAD)
- IC rare et tardive
Auscultation : parasternal D, doux, humé, aspiratif.
Roulement de Flint apexien ou bruit de galop = sténose miracle fonctionnelle, IA sévère
ECG : HVG diastolique ( R précordiales G, T neg) ou systolique (T neg précordiales G)
Signes HVD
Pronostic IA
- retentissement VG
- risque EI
- dissection aortique
Indications chirurgie IA
Suivi TDM aorte
- symptomes fonctionnels
- dilatation VG (DTS >50mm, DTD >70)
- FE < 50%
- Dilatation aorte ascendante >ou= à 55mm
Tdm ou irm aorte 1/an si IA dystrophique ou bicuspidie (sans ind chir)
Traitement médical et Indication opératoire dilatation aorte
(Marfan et Bicuspidie)
- Marfan : chirurgie aorte si
.>50mm
.>45mm et antécédents fam dissection / IA associée ou IM sévère / augmentation diamètre >2mm par an / désir grossesse - Bicuspidie : chir aorte si
.>55mm
.>50mm et antécédent fam dissec / HTA / augmentation >2mm par an
Sinon traitement médical de la dilatation aortique sans indication chirurgie (<55mm ou cf dessus) = BB ou LOSARTAN
