clinic BP of 140/90mmHg and ABPM or HBPM daytime average is 135/85mmHg or higher

-when stage 2 hypertension or -stage 1 end organ damage or -10 year CV risk >10%

- A + C or A + D (ACEI/ARB)
- Triple therapy: CCB + ACEI/ARB + thiazide diuretic
- Resistant hypertension: + low dose mineralcorticoid antagonist (spironolactone) or alpha blocker or beta blockers
