Symptoms of HUS
Nausea, diarrhoea, anorexia, abdominal pain, weaknrss, oligouria, oedema, pyrexia, hypertension, tachycardia
What causes HUS?
E. coli 0157 & Shigella - shigatoxin
What might lab findings show?
High creatinine, high urea, proteinuria, haemolytic anaemia
What might a HUS blood film show?
Schistocytes, microspherocytes, thrombocytopenia, reactive neutrophilia
What does shiga toxin do?
damage intestinal epithelium and gain access to vascular system, bind to endothelial cells causing apoptosis, necrosis, induce inflammation and activate platelets. cause thrombotic microangiopathy - TMA causes macroangiopathic haemolytic anaemia sue to RBC fragmentation causing mechanical trauma
Why does it affect kidneys?
Toxin receptors on glomerular endothelial cells
Treatment of HUS?
RBC & PLT transfusion & rehydration therapy
Are antibiotics use to treat HUS?
No they can worsen symptoms - used if risk of sepsis
Is this acute or chronic kidney disease?
acute - 10% develop chronic - 25% have long-term damage
