Pathophysiology?

Stages and symptoms of HIV?
1.Acute infection-asymptotic
2.seroconversion- 3-12w after infection. Symptomatic in 60 to 80% .glandular fever like illness.
Features- sore throat, lymphadenopathy, malaise, myalgia, athralgia, diarrhea, mucopapular rash, mouth ulcers, rarely meningoencephalitis. Ab to HIV may not be present. HIV PCR and p24 Ag can confirm the diagnosis
3.later- constitutional symptoms- fever, night sweat, diarrhea, weight loss, minor opportunistic infection, oral candida, oral hairy leukoplakia, herpes zoster, recurrent HSV, seborrhoeic dermatitis, tinea
AIDS- presence of indicator disease in the presence of HIV infection or CD4+< 200X10 6 /L
Diagnosis?
Screen - Serum HIV Ab by 4th gen ELISA ( ab,p24 Ag which can be detected by 2 weeks)
Confirm - western blot
Recent infects (window period is 1-3 w) -p24 ag or viral RNA
Complications of HIV ?
Respi 4 Kaposi sarcoma Gut 6 Eye 1 CNS
Tx?
HAART (atleast 3 drugs)
