Hair Flashcards
In utero, babies molt body and scalp hair when?
22 and 28 weeks
Hair in the Anagen phase does not come out, it is rooted very deeply. How long does this growth phase last?
1-5 years
Hair in the Telogen phase does come out, its root is closer to the surface. This phase lasts
3 months
What is androgentic alopecia
Common ass baldness
What is the mechanism of androgentic alopecia
Gradual minituriazation of the hair. Transformation of large terminal follicles to small vellus follicles. The hair becomes basically invisible. SO with common baldness, you’re not actually losing hair as much as the follicles and hair are just becoming tiny.
Male pattern androgentic allopecia
frontotemporal recession and vertex thinning, loss of all scalp hair exceptociptial fringe
Femal pattern baldness
women tend to just lose the hair from their crowns
female pattern baldness usually starts when?
25-35
Lab tests for male pattern androgentic allopecia appearing in women
DHEAS and testosterone levels
The inheritance pattern for androgentic allopecia is…
autosomal dominant or multifactorial. can come from anywhere.
What is the moa of minoxidil?
Increases hair density either by induction of anagen stage or decrease in anagen stage duration. hair diameter is increased.
No one is completely sure how it works
Doctor in miami,,,,bedside observation that led to treatment
2% minoxidil for
women
5% for
men
WHAT HORMONE CAUSES MALE PATTERN LOSS IN GENETICALLY SUSCEPTOBLE INDIVIDUALS
5 ALPHA DIHYDROTESTOSTERONE….KNOW THIS
HOW DOES FINESTERIDE WORK?
IT INHIBITS 5 ALPHA REDUCTASE WHICH MAKES 5 ALPHA DIHYDROTESTOSTERONE
The sexual side effects of finesteride can last how long
forever, according to a 2012 FDA study
TELEGEN EFFLUVIUM
PREMATURE SHUNTING OF ANAGEN TO TELOGEN. RESULTS IN MASS LOSS OF 20-35% OF TOTAL HAIR AT ONCE.
When is Telogen effluvium seen?
Usually 6 weeks to 3 mths after the precipitating cause. Precipitating causes can be:
childbirth is a big one
surgery
certain drugs
- the prototype is about 3mths after pregnancy some woman starts losing all of her hair
It usually regrows in 6-12 mths unless the precipitating cause is repeated.
Treatment of telogen effluvium
reassurance and rogaine
What is anagen efflucvium
alteration in the growth of anagen hair….90% hair loss about 3 weeks after teh cause
What is the precipitating cause of anagen effluvium?
Usually chemotherapy
ALOPECIA AREATA
CIRCULAR AREAS OF COMPLETE LOSS WITH NORMAL SCALP
- THIS IS IMMUNOLOGICALY MEDIATED
- IMPORTANT!! biopsy shows swarm of bees, lymphocytes around hair bulb
- hair usually regrows, also usually accompanied by pitting
The main concern with traction allopecia is…
it can result in staph aureus folliculitis
Folliculitis is almost always caused by
staph
Moth eaten appearance
secondary syphillis
RPR positive
syphillis
Does hair regrow when you treat someone with syphillis
yeah, usually
IF A PT COMPLAINS OF HAIR LOSS, YOU SHOULD:
CHECK THE SCALP FOR DISEASE
ORDER A: CBC TO CEHCK FOR ANEMIA
ORDER FERRITIN TESTS TO CHECK FOR LOW FERRITIN (IF THE CBC IS NORMAL AND FERRITIN IS LOW THEN GIVE IRON), TSH (HYPO AND HYPER-THYROIDISM
