What is stage 1 Cervical cancer?
Carcinoma strictly confined to the cervix
What is stage 1A Cervical cancer?
Microscopic lesion less than or equal to 5mm of depth
What is stage 1B Cervical cancer?
Clinical lesion >5mm depth of invasion
What is stage 2 Cervical cancer?
Limited to upper 2/3 vagina without parametrial involvement
What is stage 2A Cervical cancer?
Beyond uterus, upper 2/3 vagina, not to pelvic wall
What is stage 2B Cervical cancer?
Extension to parametrium but not pelvic wall
What is stage 3 Cervical cancer?
Lower 1/3 vagina, entend to pelvic wall, +/- kidney affected
What is stage 3A Cervical cancer?
Lower 1/3 vagina, not to pelvic wall
What is stage 3B Cervical cancer?
Pelvic sidewall, +/- hydronephrosis or nonfunctioning kidney
What is stage 3C Cervical cancer?
Lymph nodes
What is stage 4 Cervical cancer?
Beyond true pelvis
What is stage 4A Cervical cancer?
Spread to adjacent pelvic organs
What is stage 4B Cervical cancer?
Spread to distant organs
What is the staging workup for cervical cancer?
Staging system is clinical
1. Physical exam
2. Cervical biopsy
3. endoscopy
4. Imaging
5. Pathology
What is the treatment for stage 1A1 cervical cancer without LVSI?
Cone+simple hysterectomy
What is the treatment for stage 1A1 cervical cancer with LVSI?
Radical hysterectomy + nodes
What is the treatment for stage 1A2 cervical cancer?
Radical hysterectomy + nodes
What is the treatment for stage 1B-2A cervical cancer?
Radical hysterectomy + nodes
or
Radiation + cisplatin
What is the treatment for stage 2B-4B cervical cancer?
Radiation + cisplatin
What is a radical hysterectomy?
Removal of uterus, cervix, tissue around the cervix and upper part of vagina
What is a modified radical hysterectomy?
Removal of uterus, cervix and upper 1/3 of vagina
If a patient is not a surgical candidate for cervical cancer what is the recommended treatment?
Chemoradiation
How do you manage carcinoma of the cervix in pregnancy?
1st trimester= no change
2nd trimester= Terminate and treat
3rd trimester= Consider risks/benefits of prematurity vs delayed treatment
What are the high risk HPV subtypes?
16,18,31,33,45,52,58
Which high risk HPV subtype has the highest oncogenic potential and what is that potential?
