Indications
- Chest pain in ACS
- HTN in ACS
- Autonomic dysreflexia
- Preterm labour
- LVHF
Contraindications
- Patch SBP <90
- Tablet SBP <110
- Viagra or Levitra within last 24 hrs or Cialis within last 4 days
- HR <50
- HR >150
- VT
- Right MI
- Inferior STEMI
- Known hypersensitivity
Precautions
- No previous administration
- Elderly or frail
- Recent MI
- Current use of tocolytics
Side effects
- Hypotension
- Headache
- Tachycardia
- Bradycardia
- Skin flushing
Pharmacology
GTN is a vascular smooth muscle relaxant and is converted into NO in the body where it….
This causes dephosphorylation of the myosin light chain. Venous dilation which promotes venous pooling and reduces preload and arterial dilation which reduces systemic vascular resistance and arterial pressure, decreasing after load. The above results in reduced myocardial O2 demand, reduced SBP and DBP, mild collateral coronary artery dilation and mild tachycardia. I’m preterm labour GTN causes uterine quiesence.

Duration - 15-30mins
Patch effects
Onset - up to 30 mins
Peak - 2hrs

Metabolised by the liver
