What is Goodpasture’s syndrome?
Autoimmune disorder with antibodies generated to components of the glomerular basement membrane, affecting the kidneys and the lungs
Describe the pathophysiology of Goodpasture’s syndrome
Antibodies against alpha-3 subunit of type IV collagen produce a type II hypersensitivity reaction
What are some predisposing factors of Goodpasture’s syndrome?
- Association with HLA-DR2
- Exposure to organic solvents or hydrocarbons
- Smoking
- Infection, e.g. influenza A2
- Cocaine inhalation
- Exposure to metal dusts
What are some non-specific symptoms of Goodpasture’s syndrome
malaise, weight loss, fatigue, fever, chills, arthralgia
What are some lung symptoms of Goodpasture’s syndrome?
haemoptysis, chest pain, cough, SOB
What are some kidney symptoms of Goodpasture’s syndrome?
haematuria, proteinuria, oedema, high urea in blood, hypertension
What investigations are required in Goodpasture’s syndrome?
- Bloods - anti-GBM antibodies
- Renal biopsy - glomerulonephritis
How is Goodpasture’s syndrome managed?
Plasmapheresis
